Federation of Badminton Clubs 2019-2020 Club Affiliation and Insurance

To affiliate with Federation of Badminton Clubs, download and complete the entire FBC Affiliation & Insurance Request Form 2019-2020

Once completed, please print and mail a copy with fee to:

Some reminders about affiliation:

All recreational clubs must have mandatory Errors & Omissions/Directors & Officer’s Coverage at $40.00.

The three levels (S1, S1R and 1R) are not eligible to buy any Badminton Ontario Player’s card.

Please remember that a club can only choose one level of affiliation. If you have players in your club that are going to compete in Badminton Ontario sanctioned events, you must register as a competitive club in order for them to buy a player’s card. You can no longer affiliate as a recreational club at Level 1R and then pay $10.00/player in Level 2.

If you are a recreational club and want to buy Sport Accident Insurance, you must buy it for all of your members. Any club that buys this insurance must provide a list of the members names you purchased it for and their gender.

As a competitive club, you have the option of buying Sport Accident for your players that do not buy a player’s card. Members that buy a player’s card through Badminton Ontario are automatically covered. You also have the option of buying Errors & Omissions/Directors & Officer’s Coverage for $40.00

Upon receipt of this form and fee by YDBA (FBC), your club agrees to affiliate with YDBA (FBC). You also agree to meet the requirement of this insurance policy and the government by providing a list of your club player names and their gender and periodically providing an updated list. You understand that only named players may make a claim under this policy.
